Hi Roger:
In this case, I believe the character 命 refers to the giving of a name and 字 is a name that men take when they reach maturity or sometimes at marriage. If you read the characters at the top in the traditional order of right to left, the translation would be something like "the 字 given to 齊發." The two characters written vertically, 宗瑞, make up the name. In other words, this is probably a plaque that shows your uncle's second name, 宗瑞, which he may or may not have used much during his life. Note that this second name may be the one containing a character from the "name poem" indicating the generation to which your uncle belongs. Every generation in a branch of the clan has its own character in this poem, which could have been written hundreds of years earlier.

Thank you Greg for shedding light into this mystery. Yes, I believe you are correct. I can see 宗 in my clan's generational poem and his ancestors follow this generation naming scheme with their names.

Andy, his surname was Chin 陳 born in Canada in 1912.

No, you must be remembering someone else. My Uncle moved to California and was laid to rest there.
